I've been thinking along very similar lines. As well as the kind of responsibility that you're talking about, I think we should lean into radical accountability as a community.

When something bad happens somewhere in our community, we should each reflect on how it happened and how it might have been avoided and then try to adjust our own conduct accordingly.
